Healthwatch Newcastle Hospital Discharge Survey 2024 - This consultation has now ended

Who is the survey for?
Healthwatch Newcastle wants to hear from people who have been discharged from hospital on one of three pathways - 1,2 or 3 - in the past 12 months. If you have not been discharged from hospital in the last 12 months, or if you have been discharged and immediately returned to your own home (your 'place of residence'), this survey does not apply to you.

Who are Healthwatch Newcastle?
Healthwatch Newcastle is an organisation which is here to get the views of people in Newcastle on local health and social care services. Their goal is to improve health and social care services, show that when people share their experiences, they can make change happen, and offer people practical services, support, and advice to improve services. What is the goal of this survey?
Healthwatch Newcastle wants to evaluate the impact of being discharged from hospital on people's health outcomes, satisfaction and quality of life. This will help them understand what is working well, understand what areas may need to improve, and work towards improving services. In this survey they want to hear from people who were discharged from hospital in the last 12 months in one of three ways: 
  1. Home with support: When someone returns home and receives support from health and social care services, such as personal care, cooking and shopping. 
  2. Community recovery: When someone needs rehabilitation in a recovery unit to help them recover.
  3. Assessment of long-term care needs: When someone is discharged to a temporary care home stay whilst their long-term care needs are assessed.

More details about the 'discharge pathways':
Here's more detail about the different discharge pathways, so you can identify which one applies to you:
  • Pathway 0 - Return to usual place of residence: A volunteer from the home-from-hospital service may assist the patient in returning home and settling in, typically including a follow-up phone call to check on their recovery. The survey does not apply anyone discharged on this pathway.
  • Pathway 1 - Home with support: NHS staff will collaborate with health and social care providers to ensure initial support for the patient, like self-care, cooking, and shopping, is available when a patient returns home. The discharge team will assist until an assessment for long-term care is made. Free initial care eligibility will be assessed, while long-term support may require a financial assessment, with possible patient contributions.
  • Pathway 2 - Community recovery: Patients may need rehabilitation to reach their full potential and can be offered four types of recovery in a bedded setting, depending on their care needs (such as learning new skills, building stamina, equipment assessments, or medical supervision). Recovery units support those unable to return home due to safety concerns, such as needing home adaptations or care packages. Stays in recovery units are short-term, with patients expected to return home within four weeks.
  • Pathway 3 - Assessment of long-term care needs: If returning home isn't suitable, a temporary care home stay may be needed for recovery while long-term care needs are assessed. A social worker will assess the support required for returning home, but for those with very high needs, a permanent care home may be considered. Patients can either stay in the temporary placement or choose another care home. A financial assessment will determine any contributions towards care home costs.
Remember, this survey is for Newcastle patients with experiences of discharge pathways 1, 2 and 3. If you have been discharged on pathway 0, you do not need to fill out this survey.
